There are many different kinds of prescriptions for contacts. The prices for our evaluation fee vary by the complexity of the prescription, such as astigmatism, multifocal or monovision contact lenses, or rigid gas permeable contact lenses.
The contact lens evaluation is a separate service in addition to the eye exam and takes additional time with the doctor. We will provide you with your prescriptions from your exam with us and those can be used to order your glasses or contacts. We need a signed copy of our release of records form, signed by you, to be able to make a copy of your patient records. We require 2 business days to process these requests. Our release of records form can be found right here on our website, and may be faxed to the corresponding location.
